ResourcePath has opened three COVID-19 testing sites at Glebe Elementary, Kenmore Middle and Wakefield High open weekdays 3:30 p.m. to 7:30 p.m. with Saturday hours 9 a.m. to 5 p.m. at Kenmore Middle.  No appointment is necessary. 

Testing is free. We offer mid-nasal swab RT-PCR with results in about 24 hours.  We will use secure email to send you a formal report with your test results.

Signing up for testing is easy.  You only have to fill out the consent and intake form once using the links to the right.

What do I need to know to sign up for free testing? 

ResourcePath laboratory is asking students, family members and teachers/staff to sign a consent for testing and fill out an intake form. For children under 18, we request that a parent/guardian fill out consents and intake forms (one for each child). You will only have to fill out the consents and intake forms once. You/your child can then access the testing at any time. All information is protected under HIPAA. All testing is free with no out of pocket charges regardless of whether or not you are covered by private insurance, Medicaid, Medicare or are uninsured.

The Intake Form provides ResourcePath (the lab) with demographic and insurance information.  For those without insurance, the Social Security Number of the person being tested, if available, can be used by ResourcePath to request reimbursement for the test from HRSA, a governmental source of payment.

The Consent Form will ask you to give the laboratory permission to 

    1. Provide the test to you or your child 
    2. Release the results of testing to you by secure email, and
    3. Release the results of testing to school public health in order to assist with contact tracing 

How to sign up for testing

It is important to note that a consent and intake MUST be completed for EACH person being tested, i.e. if you have two children, you will fill out one form for each child (two forms total).   To access the forms, click one of the links on the right hand side of the page.

Upon opening the link, you will be asked to enter a name and email address. If you don’t have an email address, you can enter your mobile phone number.  If you do not have either an email address or mobile phone, please reach out to your school so that we can help you access the testing.

Parents or guardians will complete forms for students under 18. Parents or guardians filling out the form for minors should use the student’s name to complete the form, as the Intake Form will be auto populated with that information. The next screen requires parents or guardians to input their name as the person completing the form on behalf of the student. The email provided will be the one used to receive the test results.

The Consent Form is located at the top for review and signature. Please be sure to open this form, review and click the box that allows you to electronically sign the Consent Form.
